Approval to renew “C #22-89 Employee Drug Tests and Physicals” with the following companies:
• Heights Family Practice of Pearland, Texas per the terms, conditions and pricing of the current contract which is set to expire on August 22, 2024.
• MyCHN Lake Jackson of Lake, Jackson, Texas with renew of a 11.54% to 25% price increase due to inflation and escalating rental and supply costs, per the attached request.
• Next Level Urgent Care of Lake Jackson, Texas with renew of a 12.50% to 16.50% price increase, due to increases in medical supplies and labor, per the attached request.
Additionally, per the request of the Human Resource Department, add Hepatitis A,B and C panel and HIV panel.
The contract with Options Urgent Care & Wellness Center of Lake Jackson, Texas will not be renewed due to clinic closure.
The renewal term shall be August 23, 2024 to August 22, 2025.
