Approval to renew “C #22-89 Employee Drug Tests and Physicals” with the following companies per the terms, conditions and pricing of the current contract which is set to expire on August 22, 2025:
• Heights Family Practice of Pearland, Texas
• MyCHN Lake Jackson of Lake Jackson, Texas
In addition, renew with a price increase as per the attached, with Next Level Urgent Care of Lake Jackson, Texas, due to increases in medical supplies and labor.
Per the request of the Human Resource Department, add Department of Transportation Pre-Employment (DOT PE) and Hair Follicle testing.
The renewal term shall be August 23, 2025 to August 22, 2026.
